This document should be used when your employer tries to coerce you to get a vaccine or threatens to fire you. This of course is 100% illegal and opens them up serious legal consequences. Provide this document to them for them to sign, which they will not but ti will inform them of all the laws they are breaking and the liability it opens them up to.
